Contact dermatitis is a non-infectious inflammatory skin disease caused by an offending substance localized at the site of contact. The prevalence and incidence are increasing throughout the world, especially in Europe and North America. There are two main groups irritant contact dermatitis and allergic contact dermatitis with the former being more frequent. However, more than 4,000 contact allergens are known to cause allergic contact dermatitis in populations, of which nickel is the most common. Allergic contact dermatitis is a delayed IV type hypersensitivity resulting from cutaneous contact with a specific allergen. In pathogenesis, there is a significant difference between irritant and allergic dermatitis. Irritant contact dermatitis is a nonimmunologic, nonspecific inflammatory reaction to a wide range of chemical, physical or mechanical hazardous causes involving the innate immune system without prior sensitization. On the other hand, in a delayed type of hypersensitivity, concomitant activation of both the innate and adaptive immune systems is required. Recent studies disclose the role of TH17 in the pathogenesis of allergic contact dermatitis. Despite a distinct pathogenesis of both irritant and allergic contact dermatitis, there is no absolute visual distinction between both types of contact dermatitis. In general, irritant contact dermatitis tends to be milder than its allergic counterpart. Patch testing is a standardized diagnostic procedure used to confirm the causative allergen. The most common technique is an occlusive patch test, but in exceptional circumstances a modification of the patch test such as a photopatch test, open test, repeated open application test (ROAT), strip patch test or atopic patch test is used. Irritant contact dermatitis does not possess special testing. The diagnosis is therefore established by exclusion of negative test results in patch testing. Patch test may also be performed in infants and children, whereby the indications and testing technique are the same as in adults. In the management of contact dermatitis, the keystone is to cease causal exposure, meaning avoiding contact with the offending substance. In topical therapy, the choice of active substance is important, but a suitable vehicle disposes transportation to the site of inflammation. Selection of the vehicle depends on the clinical appearance and course of disease. The choice of an unsuitable vehicle may result in the drop-out of topical therapy. Topical corticosteroids are the mainstay therapy. Calcineurin inhibitors are employed in dermatology as an alternative therapy to topical corticosteroids with a better safety profile. Systemic treatments are used in severe cases and may provide temporary remission, but are not always suitable for prolonged use due to adverse effects. Alitretinoin is used to treat severe and refractory chronic hand dermatitis. After clinical clearance of contact dermatitis, the skin barrier requires restoration therapy provided by a moisturizing agent, which should be used for a prolonged period. The application of a moisturizing agent promotes epidermal barrier healing, prolongs the time to flare and reduces the number of flares. Most frequently, absolute avoidance of the triggering offending contact is very difficult, or even impossible. Therefore, protective measures to prevent renewed skin contact are indicated. Any type of symptomatic treatment cannot substitute this approach.

Numerical Modeling, Prediction of Ship Maneuvering and Hydrodynamics during Inland Waterway Transport - Peng Du - Bog - Nova Science Publishers Inc -

In this work, ship hydrodynamics during inland waterway transport and ship manoeuvring are investigated using CFD (Computational Fluid Dynamics) based on OpenFoam. Validation and verification studies are carried out for the mesh convergence, time step convergence, sensitivity to turbulence models and dynamic mesh techniques. A quaternion-based 6DoF motion solver is implemented for the trim and sinkage predictions. Environmental effects on several inland vessels (convoy 1, convoy 2, tanker) are studied using the validated numerical models. Three important aspects, the confinement effect of the waterway, head-on encounter, and ship-bridge pile interaction are simulated. The testing conditions cover a wide range, including various channel dimensions, water depths, ship draughts and speeds. The ship resistance, wave pattern, Kelvin angle and wave elevation at specific positions are investigated as functions of these parameters. Ship manoeuvring is investigated using virtual captive model tests based on the MMG (Mathematical Maneuvering Group) model. An actuator disk is implemented to replace the real propeller. Open water test, rudder force test, OTT (Oblique Towing Tank test) and CMT (Circular Motion Test) of a KVLCC2 model are carried out to obtain the hydrodynamic coefficients of the propeller, rudder and ship hull. Using the obtained coefficients, system-based manoeuvring simulations are carried out and validated using the free running test data. These studies reproduce real ship tests and thus prove the validity of our numerical models. As a result, the numerical solver is promising in ship hydrodynamics and marine engineering simulations.

The bubonic plague, smallpox, HIV/AIDS, malaria, and influenza are some of the worst epidemics/pandemics that afflicted human beings from antiquity to modern times, killing millions of people. The mosquito-borne human-infecting viruses, rotavirus, and the seasonal influenza regularly affect lives across continents in the millions. Currently, we are facing various disease-inducing organisms of the Flavivirus family the Yellow fever virus (YFV), West Nile virus (WNV), Japanese encephalitis virus (JEV), Tick-borne encephalitis virus (TBEV), Dengue virus (DENV) and Zika virus (ZIKV). Zika virus was first isolated from a febrile rhesus monkey in the Zika forest of Uganda in 1947. It drew out heightened attention following the large number of microcephaly in newborn babies in Latin America in 2015. On 1 February 2016, WHO declared that the association of Zika infection with clusters of microcephaly and other neurological disorders constituted a Public Health Emergency of International Concern. Although the severity and number of ZIKV afflicted cases have gone down lately, the public health community worldwide is keeping a watchful eye on it. ZIKV is an enveloped virus with a positive-sense RNA genome that translates into a single polypeptide, which then gets cleaved into three structural and seven nonstructural viral proteins. As compared to the earlier centuries, we have an expanding repertoire of diverse and fast acting technologies available at our hand to manage and mitigate emerging diseases. Completion of the Human Genome Project has given rise to sequencing methods which can quickly determine the genetic sequence of an alarming pathogen. Following Moores law, the power of computers, essential for data analysis and visualization, has grown substantially with time. The combination of genomic data of pathogens, computational modeling methods, and data driven methodologies can lead to a generic approach of surveillance, mitigation, vaccine design as well as new drug discovery for emerging pathogens. The four pillars of this may consist of: a) Epidemiological approaches for the characterization of carriers of next possible emerging pathogens; b) Fast computational sequence comparison methods for the characterization of emerging pathogens to understand how novel or severe they could be; c) Once the sequences of the pathogen have been determined, computer-aided vaccine design (CAVD) methods can be used to suggest a set of probable vaccine candidates for quick synthesis and testing in the laboratory; and d) Computer-assisted design of novel therapeutics and their laboratory testing as well as repurposing of already existing FDA-approved drugs. A combination of different classes of methods in vivo, in vitro and in silico will be needed to tackle suddenly emerging viruses like the Zika virus. As in many other fields of biomedical research, high throughput and low cost in silico technologies may provide the first line of defense against such emerging pathogens providing decision support systems for the judicious applications of medium throughput in vitro methods and low throughput as well as costly in vivo techniques. An emerging trend in many scientific disciplines is a strong tendency of being transformed into some form of information science. The editors of this book sincerely hope that the four-pronged computer-assisted approach described in this book will help in managing and mitigating emerging infectious diseases like Zika.

Over the past two decades, it has been recognized that advanced image processing techniques provide valuable information to physicians for the diagnosis, image guided therapy and surgery, and monitoring of human diseases. This book introduces novel and sophisticated mathematical problems which encourage the development of advanced optimisation and computing methods, especially convex optimisation. The authors go on to study Steffensen-King-type methods of convergence to approximate a locally unique solution of a nonlinear equation and also in problems of convex optimisation. Real-world applications are also provided. The following study is focused on the design and testing of a Matlab code of the Frank-Wolfe algorithm. The Nesterov step is proposed in order to accelerate the algorithm, and the results of some numerical experiments of constraint optimization are also provided. Lagrangian methods for numerical solutions to constrained convex programs are also explored. For enhanced algorithms, the traditional Lagrange multiplier update is modified to take a soft reflection across the zero boundary. This, coupled with a modified drift expression, is shown to yield improved performance. Next, Newtons mesh independence principle was used to solve a certain class of optimal design problems from earlier studies. Motivated by optimization considerations, the authors show that under the same computational cost, a finer mesh independence principle can be given than before. This compilation closes with a presentation on a local convergence analysis for eighthorder variants of HansenPatricks family for approximating a locally unique solution of a nonlinear equation. The radius of convergence and computable error bounds on the distances involved are also provided.

From its earliest forerunners in Egyptian mummification and the influence of Herophilus and Galen in ancient Greece, the autopsy has deep roots in the historical effort to understand the human body. Advances in modern post-mortem examination technique, such as non-invasive imaging, will continue to shape the structure and role of the autopsy as it evolves and changes into the future. The autopsy is a major aspect of the practice of medicine which is used to audit the effectiveness of clinical practice and assist the law courts in the adjudication of cases in which deaths occurred in suspicious circumstances in order to guarantee a safe society, prevent secret homicide, premature deaths and avoid miscarriage of justice. Practical, legal and ethical aspects of the molecular autopsy method are also discussed. Early diagnosis by genetic testing will force lifestyle modifications in individuals with genetic risk factors, which alone or in combination with other therapeutic options may delay the onset of the disease. On-site examinations at autopsy in forensic practice are discussed. Since such "on-site" examinations are simple and not time-consuming, the results can be obtained promptly and may be useful for forensic diagnosis. Autopsy rates have dramatically declined in the last several decades. As such, the authors explore the myriad of aspects that may be contributing to this downward trend. At a societal level, autopsies play a crucial role in public health and the justice system. They are necessary in understanding the causes and course of epidemic outbreaks and recognizing the emergence of new diseases. The goal of the concluding chapter is to examine religious beliefs around death and common reasons why religion may be invoked when deciding not to consent to an autopsy. Religions that will be examined include Judaism, Islam, Christianity, Christian Science, Church of Jesus Christ of Latter Day Saints, Jehovah's Witness, Hinduism, and Buddhism.

In the past migrations opened the world to knowledge, science, and understanding among peoples, but increasingly migrants are viewed with disfavour and even hate, especially in the Mediterranean Basin and the MENA Region. I had published with two other scholars a book intended to promote the rights of migrants in 2015, but today the situation has become so much worse, particularly in our chosen area, that I believe the time has come to re-examine the situation. A number of issues conspire to render the fate of migrants truly desperate, as they are forced to leave their lands that, due to the increasingly grave effects of climate change, can no longer feed and support them and their families, while the ongoing conflicts in the area render their situation truly unliveable. In addition, in the last few years, extreme right-wing political parties in Europe have conspired to treat people of different colour, ethnicity, or religion as unworthy of the respect due to all human beings. In 2019 a further disaster struck the whole world, a pandemic that imposed particularly harsh conditions to migrants, who were clearly unable to practice social distance and who found themselves in situations where testing, medicines and even sufficient food and water were not available. There are important aspects of this work that have global applications beyond the focus area we have chosen, such as the spread of racism, the fostering of conflicts to advance the interests of powerful countries, the dangerous spread of populism with fascist tendencies, and the spread of imperialism. Most of all, there is a lesson to be learned: walls to exclude and separate people provide no solutions for any of the grave problems we all face. Such problems can be solved all together or not at all; what is needed is a concentrated effort to acknowledge our need for each other, as only a belief in the true brotherhood of all can help.

Michel de Montaigne invented the literary term "essay" derived from the French word essai, meaning to put on trial. In his collection of essays he describes his life''s work in testing his responses to different subjects and situations, using his ego and alter ego as council for and against the case. In one such essay he writes, "Why do doctors begin by practising on the credulity of their patients with so many false promises of a cure, if not to call the powers of the imagination to the aid of their fraudulent concoctions?" It is hard to believe that this was written over 400 years ago, yet this book of essays in the style invented by Montaigne, is still addressing the same follies ascribed to 16th Century French citizens. In 1764 Voltaire published his Dictionnaire philosophique in which he took the essay format one step further by adding his sardonic wit, to better illuminate the follies and fallacies of that époque. One of his aphorisms that resonates 250 years on, went like this: "Faith consists in believing when it is beyond the power of reason to believe. It is not enough that a thing be possible for it to be believed". Thomas Browne, an English essayist of the same period attempted to understand the follies of mankind and their capacity of making "vulgar errors" in observation and belief. One was entitled "That a man hath one Rib less than a woman". Christian orthodoxy of the day taught a fundamentalist interpretation of the Bible. It therefore followed that if Eve were fashioned from Adam''s rib, then Eve''s descendents would always have one more rib than Adam''s descendents. Browne doubted that and went to study anatomy in the Low Countries and made his business to count the number of ribs on both sides of the chest in male and female cadavers.
