Perioperative Pain Management - - Bog - Oxford University Press Inc - Plusbog.dk

Millions of procedures, surgical and non-surgical, are performed around the world each year. Management of perioperative pain is of great importance to patients, and a critical management issue for physicians and other health professionals who provide perioperative care. Healthcare facilities and national accrediting organizations have established standards surrounding proper management of perioperative pain. There is now an increased burden on every hospital and training program to ensure that healthcare providers understand the essentials of pain management and are able to recognize and treat pain in a timely fashion. Poorly controlled pain leads to patient dissatisfaction and contributes to increased morbidity and mortality, such as myocardial infarction, pneumonia, and emotional effects. Proper management, including appropriate pharmacologic management and regional analgesic techniques, can improve function and shorten length of hospital stay. Patients who are undergoing procedures under sedation or anesthesia need to receive adequate pain relief with drugs or regional anesthetic techniques. In the recovery room following the procedure, the staff taking care of the patient needs to be properly trained to diagnose and treat post-procedural pain. Part of the Oxford American Pain Library, this concise, evidence-based clinical guide serves as a tool for every clinician who wishes to understand the basic mechanisms, pharmacology, invasive and noninvasive treatment modalities, guidelines and development of pain management protocols. The authors address new technologies, chronic pain issues, running an acute pain service, opioid and non-opioid pharmacology (including newly approved drugs), epidural and other regional anesthesia, and special populations such as pediatric patients, the elderly, and patients with a co-existing disease.

Could it be Adult ADHD? - Jan Willer - Bog - Oxford University Press Inc - Plusbog.dk

Could it be Adult ADHD? - Jan Willer - Bog - Oxford University Press Inc - Plusbog.dk

One out of every 10 adult psychotherapy clients likely has ADHD. Due to high comorbidities with depression, anxiety, substance abuse, and other mental disorders, along with considerable behavioral dysfunction, adult ADHD is even more common in clinical populations than the general public. When an ADHD diagnosis is missed, psychotherapy is often frustrating and less effective.Could it be Adult ADHD? is for mental health professionals who wish to learn how to recognize, assess, and treat adult ADHD. Written in a style maximally accessible to the practicing mental health professional, this book educates early-career psychotherapists and experienced professionals alike on the disorder and its treatment. Author Jan Willer provides a full description of adult ADHD symptoms, based on the most current research, including executive functioning problems, emotional dysregulation, atypical reward sensitivity, and problems with time perception. Recognizing patterns of dysfunction is essential to identify ADHD, so two detailed composite cases are presented, along with supplemental case material. Strengths that may be associated with ADHD are described. Willer offers guidance on providing psychoeducation about cognitive differences in ADHD, which is essential for client self-acceptance and adaptive functioning. Common psychotherapy problems with ADHD clients are addressed, including chronic lateness to sessions, missed appointments, motivation problems, difficulties with homework, and tangentiality. Willer also discusses medications for ADHD, including their benefits, contraindications, and side effects, and reviews the effectiveness of non-traditional treatments.

Epilepsy - Daniel Friedman - Bog - Oxford University Press Inc - Plusbog.dk

Before Prozac - Edward Shorter - Bog - Oxford University Press Inc - Plusbog.dk

Before Prozac - Edward Shorter - Bog - Oxford University Press Inc - Plusbog.dk

Psychiatry today is a barren tundra, writes medical historian Edward Shorter, where drugs that don't work are used to treat diseases that don't exist. In this provocative volume, Shorter illuminates this dismal landscape, in a revealing account of why psychiatry is "losing ground" in the struggle to treat depression. Naturally, the book looks at such culprits as the pharmaceutical industry, which is not inclined to market drugs once the patent expires, leading to the endless introduction of new--but not necessarily better--drugs. But the heart of the book focuses on an unexpected villain: the FDA, the very agency charged with ensuring drug safety and effectiveness. Shorter describes how the FDA permits companies to test new products only against placebo. If you can beat sugar pills, you get your drug licensed, whether or not it is actually better than (or even as good as) current medications, thus sweeping from the shelves drugs that may be superior but have lost patent protection. The book also examines the FDA's early power struggles against the drug industry, an influence-grab that had little to do with science, and which left barbiturates, opiates, and amphetamines all underprescribed, despite the fact that under careful supervision they are better at treating depression, with fewer side effects, than the newer drugs in the Prozac family. Shorter also castigates academia, showing how two forms of depression, melancholia and nonmelancholia--"as different from each other as chalk and cheese"--became squeezed into one dubious classification, major depression, which was essentially a political artifact born of academic infighting. An astonishing and troubling look at modern psychiatry, Losing Ground is a book that is sure to spark controversy for years to come.

Pain Care Essentials - - Bog - Oxford University Press Inc - Plusbog.dk

Pain Care Essentials - - Bog - Oxford University Press Inc - Plusbog.dk

It may come as something of a surprise that pain, the most prevalent symptom in clinical practice, is not always addressed specifically in health professions training. Approximately one in six Americans lives with chronic pain in addition to the millions that experience acute pain each day. Half of older adults live with chronic pain-associated conditions, and about half of all healthcare visits are initiated because of pain. Despite this, reports indicate that the vast majority of health professions schools in the United States do not teach required courses on pain, and the total amount of content pertaining to pain is a fraction of a percent of the total. Almost certainly, the lack of education in coordinated, comprehensive, compassionate care for pain-associated conditions contributed to pervasive opioid over-prescribing and the ensuing wave of addiction and deaths that swept the country in the first part of this century. This book is our response to the pain care crisis - it is designed to prepare young clinicians to assess and treat a wide variety of pain conditions in a manner that balances competence and compassion, incorporating coordinated elements of pharmacological and non-pharmacological therapies. Designed to be read during or after pre-licensure training, e.g. medical, nursing, pharmacy school, and to inspire students to learn more about painful conditions, this book is unique in its clinical focus and the level of detail that is included. This book aims to improve pain care, most especially if used alongside a formal pain care course as part of pre-licensure training, whether spread over four years or condensed into a shorter period. Through engagement in the interprofessional curriculum planning process, the content of the book has been shaped to align with the International Association for the Study of Pain (IASP) interprofessional pain curriculum vision and to focus on the primary questions of: What is pain? How is pain assessed? How is pain managed? How does clinical context influence pain?

Managing Your Child's Chronic Pain - Emily F. Law - Bog - Oxford University Press Inc - Plusbog.dk

Managing Your Child's Chronic Pain - Emily F. Law - Bog - Oxford University Press Inc - Plusbog.dk

Chronic pain is a significant health problem for many children and adolescents, with studies suggesting that as many as 15% to 40% of children suffer from chronic pain. Chronic pain is puzzling for parents to understand because there is often not a clear diagnosis or treatment plan. Unfortunately, many families cannot get the help they need because they live too far from children''s hospitals with pediatric pain clinics, where most healthcare providers in this specialized area are based. Managing Your Child''s Chronic Pain is a resource for parents to learn how to help their children and families cope with persisting pain using cognitive-behavioral therapy (CBT), an effective intervention to treat children with chronic pain. With the easy-to-use psychological treatment strategies in this book, parents will have practical instructions in CBT for pain management. Authors Tonya M. Palermo and Emily F. Law, pediatric psychologists in the field of child and adolescent chronic pain, discuss the scientific foundation for chronic pain, information about the effectiveness of treatments, and several personal stories of parents and their children with chronic pain. They explain instructions for several different interventions including deep breathing, relaxation strategies, and sleep interventions to allow parents to support these skills with their children. The book concludes with guidance on how to prevent relapse, maintain improvements, and prevent future problems with pain and disability. The first book of its kind, Managing Your Child''s Chronic Pain will empower parents to take a hands-on approach to relieving their child''s pain.

Uncertainty in Medicine - Paul K.j. (senior Scientist Han - Bog - Oxford University Press Inc - Plusbog.dk

Uncertainty in Medicine - Paul K.j. (senior Scientist Han - Bog - Oxford University Press Inc - Plusbog.dk

Medical uncertainty has been with us for centuries and remains a recurrent problem for patients, doctors, and researchers alike. Yet uncertainty in health care is still poorly understood and ineffectively managed; it is generally feared and avoided rather than directly confronted. This systemic disregard of uncertainty leads us to treat medical uncertainty as a pathological condition to be cured through the pursuit of knowledge, but often further medical knowledge begets further uncertainty in kind.Uncertainty in Medicine offers an alternative, multi-disciplinary perspective on this challenging problem. Integrating insights across clinical medicine and social science, Dr. Paul Han argues that uncertainty is an essential form of knowledge to be cultivated, rather than eradicated, in medical practice. He makes the case that the paradigm of medicine should be expanded to include not only the pursuit of medical knowledge but the treatment and palliation of medical uncertainty and its effects on physicians, other health professionals, and patients. Using clear language and a textbook approach, he analyzes the nature, etiology, and natural history of medical uncertainty, and develops a conceptual framework to guide its management. By promoting a more systematic way of conceptualizing the problem, this framework can enable clinicians and patients to better address medical uncertainty, and can help make uncertainty tolerance a more central focus of medical care.Rational and reassuring, Uncertainty in Medicine forges a new path for approaching medical uncertainty by arming readers from an array of disciplines with the tools they need to diagnose, treat, and confront its challenges more intentionally and effectively.
