Cancer. pain and tenninal care. contraception. and hyperlipoproteinaemias. It also contains pieces on common emergencies. home renal dialysis and tropical diseases in non-tropical countries. The creation of ''pain c1inics·. and the possibilities for the treatment of intractable pain are innovations calculated to be more widely used as we become more aware of theirvalue. The field of rehabilitation is one of which we general physicians are on less familiar ground. To say that by us it is a neglected field is perhaps not too strong a statement. The section on Drugs concentrates on the practical side of drug administration and hence the stress is on the incompatibilities. side etfects and contraindications.

This book, which proposes a developmental view of short-term inpatient treatment for severely disturbed children, is much needed in our field. It is particularly relevant, emotionally engaging, and a pleasure to read because the writers who are the actual participants and leaders in the milieu program, discuss their own experiences with a variety of children. The principles of milieu therapy are beautifully described and its application to a diverse ethnic population of sick children is clearly delineated. With this book, Dr. Zivojnovic presents to the ophthalmic community the long awaited detailed report of his surgical concepts and operative techniques in the field of vitreoretinal surgery. It is fascinating to read how his concepts for the treatment of complicated retinal detachments evolved from the intra­ ocular use of silicone oil to a combination of vitreous surgery with silicone oil tamponade. The next step was surgical treatment of the retina itself. It takes an unconventional mind to break major taboos and to state the retinotomies and retinectomies are necessary if scar tissue cannot be surgically removed and that buckling procedures are no longer necessary to treat retinal breaks. This book discusses the use of silicone oil in vitreoretinal surgery. Silicone oil tamponade has been proven to be a major step forward in vitreoretinal surgery and the reported results speak for themselves. The reader may have the impression that silicone is thought to be indispensible for almost all cases undergoing this type of surgery but this modality is used to treat severe cases where the chances of success are greatly enhanced by using silicone oil. Dr. Zivojnovic remains the forerunner and great surgeon who readily admits the limitations of his techniques: ''The surgical operative treatment is only partly adequate and, unfortunately, despite the correctly performed operation, often does not lead to the desired result''.

The present volume of the Boerhaave series intends to be a reflexion of our present knowledge in the expanding field of early diagnosis and prevention of inherited disorders. Early diagnosis can mean detection of a carrier state in one or both potential parents, detection of a specific inherited disease in a previously born child or during a pregnancy at risk, or during various stages of the life of an already born individual. The first chapters will discuss inherited disorders manifesting later in life. In Huntington''s Chorea detection may be possible from the age of 20 or 30 onwards, while in myotonic dystrophy it will be seen that detection may be possible soon after birth. Retinoblastoma serves as example of a disease manifesting in early infancy and which is partially treatable by surgical intervention. The same early onset does occur in phenylketonuria and cystic fibrosis. Both diseases provide examples of the possibilities of population screening, while in phenylketonuria an eminently succesful treatment is found in a restricted diet. For the usually early manifesting sphingolipidoses no treatment exists as yet, but enzyme treatment might in the near future become a reality. Spina bifida and anencephaly are not simple inherited Mendelian dis­ orders but they are discussed in this volume because prevention is possible by antenatal diagnosis and subsequent interruption of the pregnancy if re­ quested by the future parents. Further views on antenatal diagnosis notably in inborn errors of metabolism are presented in a following chapter.

The first volume of this Handbook discussed neuroendocrine diagnostic tests and the diagnostic use of central nervous system amine metabolites. That volume further reviewed the toxicological evaluation of patients and the laboratory evalua­ tion of treatment outcome. It suggested a system for evaluating newly admitted psychiatric patients and dermed the scope of diagnostic procedures available in the emergency department. Volume II focuses on the use and interpretation of electro physiologic and radiologic diagnostic tests in psychiatry and then explores the laboratory evaluation of special groups of patients. The clinical sections of this volume are designed to assist the physician in in­ stituting a proper workup for specific patients and defining tests which will assist them in the differential diagnosis of various psychiatric disorders. Such workups are critical to exclude possible organic disorders which can present with psychia­ tric symptoms. The workup suggested for the various classes of patients will assist the clini­ cian with differential diagnosis, provide base-line information for long-term follow­ up, delineate biological perimeters at the beginning of treatment, protect the pa­ tient from unrecognized cardiac, renal, hepatic, or endocrine disorders which could be adversely affected by the administration of medications, and provide a rational sequencing of workup for particular disorders to insure the most thorough yet cost­ efficient approach to the patient.

This text outlines the fluid and thermodynamic principles that apply to all classes of turbomachines, and the material has been presented in a unified way. The approach has been used with successive groups of final year mechanical engineering students, who have helped with the development of the ideas outlined. As with these students, the reader is assumed to have a basic understanding of fluid mechanics and thermodynamics. However, the early chapters combine the relevant material with some new concepts, and provide basic reading references. Two related objectives have defined the scope of the treatment. The first is to provide a general treatment of the common forms of turbo machine, covering basic fluid dynamics and thermodynamics of flow through passages and over surfaces, with a brief derivation of the fundamental governing equations. The second objective is to apply this material to the various machines in enough detail to allow the major design and performance factors to be appreciated. Both objectives have been met by grouping the machines by flow path rather than by application, thus allowing an appreciation of points of similarity or difference in approach. No attempt has been made to cover detailed points of design or stressing, though the cited references and the body of information from which they have been taken give this sort of information. The first four chapters introduce the fundamental relations, and the suc­ ceeding chapters deal with applications to the various flow paths.

With the emergence of the biological sciences in the seventeenth and eighteenth centuries, some explanations of congenital malformations with a basis in scientific fact were formulated. It was not, however, until the thalidomide tragedy in 1961 that the necessity for further study of the effects of environmental factors on congenital malformations was recognized. By the early 1970s concentrated efforts had begun to be directed toward evaluating the effects on the unborn child of mood-altering drugs taken during pregnancy. Over the past decade, research and treatment programs have led to the recognition of a full range of psychoactive drugs that affect the developing fetus either somatically or behaviorally. Programs for the treatment of the pregnant drug addict have been developed which concentrate not only on the pharmacologic therapy of the woman but also on the social and psychologic characteristics that place her and her unborn child at risk. This book is an attempt to bring together clinicians and researchers who have been active in developing programs for the recognition and management of the chemically dependent pregnant woman and her newborn. The contributions of the multiple disciplines which the authors represent emphasize the need for a multifactorial approach to the problems of drug use and abuse during pregnancy. One of the eXCltmg challenges of medicine has been the reaching of decisions based on less than complete evidence. As undergraduates in teaching hospitals future physicians are taught to think in clear and absolute black and white terms. Diagnoses in teaching hospitals all are based on supportive positive findings of in­ vestigations. Treatment follows logically on precise diagnosis. When patients die the causes of death are confirmed at autopsy. How very different is real life in clinical practice, and particularly in family medicine. By the very nature of the common conditions that present diagnoses tend to be imprecise and based on clinical assessment and interpretation. Much of the management and treatment of patients is based on opinions of individual physicians based on their personal expenences. Because of the relative professional isolation offamily physicians within their own practices, not unexpectedly divergent views and opinions are formed. There is nothing wrong in such divergencies because there are no clear absolute black and white decisions. General family practice functions in grey areas of medicine where it is possible and quite correct to hold polarized distinct opinions. The essence of good care must be eternal flexibility and readiness to change long-held cherished opinions. To demonstrate that with many issues in family medicine it is possible to have more than one view I selected 10 clinical and II non -clinical topics and invited colleagues and fellow-practitioners to enter into a debate-in-print.

This book tries to trace the course of social development in the child from birth to maturity. It assumes that the neonate comes into the world with the endowment and the need to contact others, to attach itself to the surrounding humans and by this very endowment enforces these humans to attach themselves to it. The book follows the main vicissitudes in this process and describes in short outlines some of the disturbing manifestations of failure at different ages in the smooth progress of socialization - the process by which the child adapts to the requirements of the social group of which he is a member. In accordance with recent studies the book stresses the influence on social development of the wider environment. It is no longer believed that the quality of the early mother­ child relationship exclusively determines all later develop­ ment. The book also surveys the present state of assessment and treatment methods and tries to indicate where short intervention by the developmental paediatrician might be sufficient and where more long-term intensive treatment by specialists would be needed. The author agrees with A. M. and A. D. B. Clarke (1976) that early experiences, though powerful, are not irreversible, and that with changes in environmental circumstances, and with under- 9 Social development standing of the main causal factors for the deviance, the effect of adverse early events may be modified and develop­ ment towards normal social progress may to some extent be restored.
