Gluten sensitivity is a multifactorial phenomenon. In the medical context, it is associated with symptoms that occur after the consumption of gluten-containing foods. However, not all cases of perceived gluten sensitivity are medically diagnosable. Only for celiac disease and wheat allergies clear diagnostic criteria exist. Affluent countries have become more con­ cerned with the health effects of nutrient excesses than with deficien­ cies, while hungry nations continue to suffer shortages.

The first part of this title contained all statistical tests relevant to starting clinical investigations, and included tests for continuous and binary data, power, sample size, multiple testing, variability, confounding, interaction, and reliability. The current part 2 of this title reviews methods for handling missing data, manipulated data, multiple confounders, predictions beyond observation, uncertainty of diagnostic tests, and the problems of outliers. Also robust tests, non-linear modeling , goodness of fit testing, Bhatacharya models, item response modeling, superiority testing, variability testing, binary partitioning for CART (classification and regression tree) methods, meta-analysis, and simple tests for incident analysis and unexpected observations at the workplace and reviewed. Each test method is reported together with (1) a data example from practice, (2) all steps to be taken using a scientific pocket calculator, and (3) the main results and their interpretation. Although several of the described methods can also be carried out with the help of statistical software, the latter procedure will be considerably slower. Both part 1 and 2 of this title consist of a minimum of text and this will enhance the process of mastering the methods. Yet the authors recommend that for a better understanding of the test procedures the books be used together with the same authors'' textbook "Statistics Applied to Clinical Studies" 5th edition edited 2012, by Springer Dordrecht Netherlands. More complex data files like data files with multiple treatment modalities or multiple predictor variables can not be analyzed with a pocket calculator. We recommend that the small books "SPSS for starters", Part 1 and 2 (Springer, Dordrecht, 2010, and 2012) from the same authors be used as a complementary help for the readers'' benefit.

In recent years there has been an impressive surge of interest in the mecha­ nisms which control the onset and the maintenance of labour. Much of it stems from a constant awareness that gestational age at birth is still an important factor in neonatal mortality and morbidity. Advances in the sensitivity and specificity of immunoassay, the discovery of the prosta­ glandins, new skills in fetal surgery and chronic catheterisation in experi­ mental animals have greatly stimulated further research in this area. In the meantime clinicians became continuously more aware that, despite consider­ able advances in perinatal medicine, gestational age at birth remains the major determinant of neonatal mortality and morbidity. It led them to seek new and better ways of controlling uterine function whether by influencing cervical ripeness, by stimulating or by inhibiting myometrial contractility. This volume uniquely combines knowledge which has been gained from both experimental and clinical research on parturition. A brief outline of current evidence on the control of parturition in experimental animals may be fruitful in delineating problems and hypotheses for further study in human pregnancy. Much of our present day knowledge of human parturition has been gained by checking such data against experimental findings, careful measurements and clinical observations in human pregnancy. It is hoped that the interdisciplinary nature of this volume will enhance the understanding of human parturition and stimulate further research in this area. Thanks are due to Miss H. Crises are not a feature of depressive illness; but this illness needs to be considered in the diagnoses of three acute emergencies: the agitated patient, the withdrawn patient and the suicidal patient. A. The agitated patient. Restless, anguished, phrenetic and impor­ tunate behaviour. Differential diagnoses include hypomania, acute anxiety and grief, hysteria, drug intoxication, thyrotoxicosis, cerebrovascular accident or cerebral tumour. Agitated depression carries a relatively high risk of suicide. Management usually requires admission and use of adequate doses of antidepressant and neuroleptic drugs, and often ECT. B. The withdrawn patient who avoids social contacts and obligations and is often slowed up in mind and body. Differential diagnoses in­ clude schizophrenia, CVA or tumour, hysteria and semi-coma includ­ ing drug intoxication. Withdrawn and retarded patients with depres­ sive illness are at risk of failing to eat or care for themselves. C. The suicidal patient. May present as unexpected, inexplicable coma; a badly cut patient may be confused by the doctor with acci­ dent or assault. The immediate emergency is medical or surgical: treatment is for coma, bleeding or asphyxia, and requires immediate admission to casualty. The first presentation of depression is always a minor emergency as it may be the only attempt the patient makes to see a doctor. Diagnosis must be positive, based on the recognition of depres­ sive features, not negative, based on the exclusion of other dis­ eases. The cardinal symptoms of depressive illness: 1. Disturbed sleep pattern. 2. Change in appetite for food.

For example, diesel fuel prices in some developing countries have increased, while world oil prices have decreased. The rural sector of developing countries depends heavily on noncommercial energy sources. Availability of noncommercial energy is highly site-specific.
