The advent of any new and effective therapy is soon followed by large numbers of publications in which the indications and benefits are explored critically. It is not unexpected, therefore, that within five years of the first Bermuda Symposium on advances in parenteral nutrition that a second Symposium was considered appropriate to review progress and explore new areas of investigation, as well as enlarging the scope of the meeting to include enteral nutrition. The rate of progress can be judged by the number of subjects which were not discussed at the first Symposium. For example, home parenteral nutrition, computer assisted assessment and prescribing, Studies of body protein synthesis and breakdown and the role of branched-chain amino acids are all new subjects for this Symposium which were not covered at all in the first meeting. Much progress has also been made to our understanding of the biochemi­ cal complications of parenteral nutrition and the problems related to long term access to the circulation. Nutritional care has become safer and more effective. There is an increasing awareness of the difficulties in making a true nutritional assessment in selecting patients for total parenteral nutrition and more attention has also been focussed on different approaches to enteral support in the management of undernourished patients. There is also continuing debate on the cost effectiveness of this expensive method of treatment and critics look in vain for evidence of efficacy based on controlled trials in specific groups of patients.

It is a pleasure to present, Nutrition and Exercise in Obesity Management, for reference and textbook use. The text is an outgrowth of the Obesity­ Weight Control Track of the 1982 La Crosse Health and Sports Science Symposium, sponsored annually by the La Crosse Exercise Program, University of Wisconsin-La Crosse. With versatile faculty, topics, and attending professionals, the Obesity-Weight Control Track stimulated an effort to produce interdisciplinary resources on obesity. Out of this effort, three books have been compiled and edited. The first book, Evaluation and Treatment of Obesity, introduces an inter­ disciplinary, practical approach to obesity management. This book, Nutrition and Exercise in Obesity Management, compiles the information specifically related to nutrition and exercise management of obese indi­ viduals. The third book, Behavioral Management of Obesity, relates be­ havioral theories to the modification of eating habits and activity patterns. These three books apply the latest information from the fields of medi­ cine, nutrition, exercise, and psychology to the problem of obesity. The information is intended to guide health professionals in the interdisci­ plinary management of obesity. In 1983 the Obesity-Weight Control Track focused on controversial issues of theoretical and practical concern. The speakers from this track contributed their expertise to the compilation of two additional books. Thus, Trends and Controversies in Obesity Research and Innovation in Obesity Program Development will complete the series. Consider the five volumes a consolidated, comprehensive reference related to the growing, interdisciplinary field of weight control.

The current part 2 of this title reviews multistep methods, multivariate models, assessments of missing data, performance of diagnostic tests, meta-regression, Poisson regression, confounding and interaction, and survival analyses using log tests and segmented time-dependent Cox regression. Methods for assessing non linear models, data seasonality, distribution free methods, including Monte Carlo methods and artificial intelligence, and robust tests are also covered. Each method of testing is explained using a data example from clinical practice,including every step in SPSS, and a text with interpretations of the results and hints convenient for data reporting. In order to facilitate the use of this cookbook the data files of the examples is made available by the editor through extras.springer.com. Both part 1 and 2 of this title contain a minima amount of text and maximal technical details, but we believe that this property will not refrain students from mastering the SPSS software systematics, and that, instead, it will be a help to that aim. Yet, we recommend that it will used together with the textbook "Statistics Applied to Clinical Trials" (5th edition, Springer, Dordrecht 2012) and the e-books "Statistics on a Pocket Calculator Part 1 and 2 (Springer, Dordrecht, 2011 and 2012) from the same authors.

The first part of this title contained all statistical tests relevant to starting clinical investigations, and included tests for continuous and binary data, power, sample size, multiple testing, variability, confounding, interaction, and reliability. The current part 2 of this title reviews methods for handling missing data, manipulated data, multiple confounders, predictions beyond observation, uncertainty of diagnostic tests, and the problems of outliers. Also robust tests, non-linear modeling , goodness of fit testing, Bhatacharya models, item response modeling, superiority testing, variability testing, binary partitioning for CART (classification and regression tree) methods, meta-analysis, and simple tests for incident analysis and unexpected observations at the workplace and reviewed. Each test method is reported together with (1) a data example from practice, (2) all steps to be taken using a scientific pocket calculator, and (3) the main results and their interpretation. Although several of the described methods can also be carried out with the help of statistical software, the latter procedure will be considerably slower. Both part 1 and 2 of this title consist of a minimum of text and this will enhance the process of mastering the methods. Yet the authors recommend that for a better understanding of the test procedures the books be used together with the same authors'' textbook "Statistics Applied to Clinical Studies" 5th edition edited 2012, by Springer Dordrecht Netherlands. More complex data files like data files with multiple treatment modalities or multiple predictor variables can not be analyzed with a pocket calculator. We recommend that the small books "SPSS for starters", Part 1 and 2 (Springer, Dordrecht, 2010, and 2012) from the same authors be used as a complementary help for the readers'' benefit.

The first volume of this Handbook discussed neuroendocrine diagnostic tests and the diagnostic use of central nervous system amine metabolites. That volume further reviewed the toxicological evaluation of patients and the laboratory evalua­ tion of treatment outcome. It suggested a system for evaluating newly admitted psychiatric patients and dermed the scope of diagnostic procedures available in the emergency department. Volume II focuses on the use and interpretation of electro physiologic and radiologic diagnostic tests in psychiatry and then explores the laboratory evaluation of special groups of patients. The clinical sections of this volume are designed to assist the physician in in­ stituting a proper workup for specific patients and defining tests which will assist them in the differential diagnosis of various psychiatric disorders. Such workups are critical to exclude possible organic disorders which can present with psychia­ tric symptoms. The workup suggested for the various classes of patients will assist the clini­ cian with differential diagnosis, provide base-line information for long-term follow­ up, delineate biological perimeters at the beginning of treatment, protect the pa­ tient from unrecognized cardiac, renal, hepatic, or endocrine disorders which could be adversely affected by the administration of medications, and provide a rational sequencing of workup for particular disorders to insure the most thorough yet cost­ efficient approach to the patient.

Over the past few years there have been considerable advances in our understanding of the normal development of vision and in our ability to detect and meaSl1re visual impairment in early childhood. It was appropriate, therefore, that a workshop, sponsored by the European Communities, should be held on the ''Detection and Measurement of Visual Impairment in Pre-verbal Children.'' This workshop, which was held at the Institute of Ophthalmology, London, between 1 and 3 April 1985, brought together visual physiologists and ophthalmologists who exchanged and discussed ideas of mutual interest. After an introductory session when the normal development of vision and the causes of visual impairment were reviewed, there were sessions devoted to the theoretical aspects of electrophysiological and psychophysical tests, the measurement of visual acuity in pre-verbal children, the measurement of other visual functions, and visual screening of pre-verbal children. This volume contains the papers presented at the workshop, and transcripts of the various discl1ssions that took place. It was a measure of the success of the workshop that participants from several different disciplines were able to have fruitful discussions and to suggest areas of common interest where collaborative ventures could usefully be pursued. It is hoped that this venture will be followed by others where a mul ti­ disciplinary approach will improve both our knowledge of visual handicap in childhood and our management of this important group of sensorily impaired children.
