Who Knew? Inside the Complexity of American Health Care

Who Knew? Inside the Complexity of American Health Care

Despite all the writing and the research America’s health care industry continues to fail at providing health care that is accessible and affordable with measurable quality. The fundamental reason we have failed is that health care is not only a complex business but the most complex in our economy. Other industries are disrupted some readily adapt to new markets; some leverage information technology and innovative and cost-saving ways. But to date health care has resisted. The customary approaches tried in other industries seem not to apply to health care. Why? Why is the health care industry so politically divisive? Why is the quality of health care services so difficult to measure? Why do patients often fail to understand their own health care? Why are security and privacy such unique challenges in health care? Why is the payment process for health care services so complicated and challenging? This book seeks to answer these questions. This book written by a well know industry ‘insider’ with 35+ years working at senior levels in hospital operations and information technology discusses nine major factors that in combination contribute to health care’s complexity. The author concludes that until we understand why health is so complex we will continue to see books complaining about the poor state of health care in the U. S. and proposals for change that are generally unsuccessful and innovative technology products that fail to deliver expected results. | Who Knew? Inside the Complexity of American Health Care

ISO 45001 Implementation How to Become an Occupational Health and Safety Champion

Over two million people worldwide die every year due to work-related accidents and illness which corresponds to over 6000 deaths every day (International Labour Organisation 2020). Globally there are about 340 million occupational accidents and 160 million victims of work-related injuries and illness each year (International Labour Organisation 2020). Occupational health and safety is a major challenge for many organizations. Regardless of the size and nature of their business organizations should protect their people and provide a safe and healthy working environment. They should identify the potential health and safety risks present in their workplace and take appropriate action to keep their workers free from harm. Occupational safety focuses on potential safety hazards that can cause injury. Occupational health on the other hand looks at potential health issues such as occupational medicine occupational hygiene and primary health care including the wellbeing of workers. For organizations that want to implement an occupational health and safety management system based on the ISO 45001:2018 standard but are not familiar with its structure and definitions it often takes a significant amount of resources to understand the requirements of the standard and plan their implementation. This book provides guidance in establishing an occupational health and safety management system linked to the requirements of ISO 45001:2018. It aims to explain all the requirements of ISO 45001:2018 clause by clause to provide guidance to: • Organizations preparing for ISO 45001:2018 implementation • Individuals who want to build a career in occupational health and safety • Health and safety practitioners and managers who want to improve their occupational health and safety performance • Occupational health and safety consultants who prepare their clients for ISO 45001:2018 certification audits • Internal and external auditors who audit occupational health and safety management systems. Digital Health and Patient Data Empowering Patients in the Healthcare Ecosystem

Digital Health and Patient Data Empowering Patients in the Healthcare Ecosystem

Patients with unmet needs will continue to increase as no viable nor adequate treatment exists. Meanwhile healthcare systems are struggling to cope with the rise of patients with chronic diseases the ageing population and the increasing cost of drugs. What if there is a faster and less expensive way to provide better care for patients using the right digital solutions and transforming the growing volumes of health data into insights? The increase of digital health has grown exponentially in the last few years. Why is there a slow uptake of these new digital solutions in the healthcare and pharmaceutical industries? One of the key reasons is that patients are often left out of the innovation process. Their data are used without their knowledge solutions designed for them are developed without their input and healthcare professionals refuse their expertise. This book explores what it means to empower patients in a digital world and how this empowerment will bridge the gap between science technology and patients. All these components need to co-exist to bring value not only to the patients themselves but to improve the healthcare ecosystem. Patients have taken matters into their own hands. Some are equipped with the latest wearables and applications engaged in improving their health using data empowered to make informed decisions and ultimately are experts in their disease(s). They are the e-patients. The other side of the spectrum are patients with minimal digital literacy but equally willing to donate their data for the purpose of research. Finding the right balance when using digital health solutions becomes as critical as the need to develop a disease-specific solution. For the first time the authors look at healthcare and technologies through the lens of patients and physicians via surveys and interviews in order to understand their perspective on digital health analyse the benefits for them explore how they can actively engage in the innovation process and identify the threats and opportunities the large volumes of data create by digitizing healthcare. Are patients truly ready to know everything about their health? What is the value of their data? How can other stakeholders join the patient empowerment movement? This unique perspective will help us re-design the future of healthcare - an industry in desperate need for a change. | Digital Health and Patient Data Empowering Patients in the Healthcare Ecosystem

Optimizing Emergency Department Throughput Operations Management Solutions for Health Care Decision Makers

Optimizing Emergency Department Throughput Operations Management Solutions for Health Care Decision Makers

Across the country ambulances are turned away from emergency departments (EDs) and patients are waiting hours and sometimes days to be admitted to a hospital room. Hospitals are finding it hard to get specialist physicians to come to treat emergency patients. Our EDs demand a new way of thinking. They are not at a tipping point; they are at a breaking point. Under current loads and trends they are going to begin to break and these breakdowns will be painful and ultimately dangerous to society. Recognizing that the ideal in health care is presently beyond our immediate grasp this book instead focuses on providing health care leaders with the tools they can employ to optimize the performance of EDs and thereby improve service to patients employees and communities. Written by 20 of the most progressive and successful health care reformers in the country the approaches described can be utilized to quantify improvements enhance predictability of workflow and improve staff scheduling. The data derived using these techniques can serve as powerful evidence in support of change. While a common discussion among ED professionals is the perception that many patients are not really emergency patients and could be treated in another setting at another time that argument is not germane until we as a nation elect to reform the way we chose to deliver healthcare to the underserviced. In the meantime this book provides invalauable information to help individual hospitals to retool their ED‘s. It offers new approaches that think outside of the box for all stakeholders. It also provides the statistical evidence that administrators need to make their cases for changes and added resources. It will help you forecast the demand for services and give your center an approach that will allow the ED to become a source of income rather than one that continues to hemorrhage needed limited health care funding. | Optimizing Emergency Department Throughput Operations Management Solutions for Health Care Decision Makers

The Logics of Healthcare The Professional’s Guide to Health Systems Science

The Logics of Healthcare The Professional’s Guide to Health Systems Science

Most of the current literature on healthcare operations management is focused on importing principles and methods from manufacturing. The evidence of success is scattered and nowhere near what has been achieved in other industries. This book develops the idea that the logic of production and production systems in healthcare is significantly different. A line of thing that acknowledges the ingenious characteristics of health service production is developed. This book builds on a managerial segmentation of healthcare based on fundamental demand-supply constellations. Demand can be classified with the variables urgency severity and randomness. Supply is constrained by medical technology (accuracy of diagnostics efficacy of therapies) patient health behavior (co-creation of health) and resource availability. Out of this emerge seven demand-supply-based operational types (DSO): prevention emergencies one-visit electives cure care and projects. Each of these have distinct managerial characteristics such as time-perspective level of co-creation value proposition revenue structure productivity and other key performance indicators (KPI). The DSOs can be envisioned as platforms upon which clinical modules are attached. For example any Emergency Department (ED) must be managed to deal with prioritization time-windows agitated patients the necessity to save and stabilize and variability in demand. Specific clinical assets and skill-sets are required for say massive trauma strokes cardiac events or poisoning. While representing different specialties of clinical medicine they when applied in the emergency – context must conform to the demand-supply-based operating logic. A basic assumption in this book is that the perceived complexity of healthcare arises from the conflicting demands of the DSO and the clinical realms. The seven DSOs can neatly be juxtaposed on the much-used Business Model Canvas (BMC) which postulates the business model elements as value proposition; customer segments channels and relations; key activities resources and partners; the cost structure; and the revenue model. | The Logics of Healthcare The Professional’s Guide to Health Systems Science

Health Information Technology Evaluation Handbook From Meaningful Use to Meaningful Outcomes

Health Information Technology Evaluation Handbook From Meaningful Use to Meaningful Outcomes

Governments and clinical providers are investing billions of dollars in health information technologies (HIT). This is being done with the expectation that HIT adoption will translate into healthier patients experiencing better care at lower cost. In the initial push to roll out HIT the reliability of these claims was often not substantiated by systematic evaluation and testing. As the first wave of widespread adoption of HIT comes to an end and the next wave begins it is more important than ever that stakeholders evaluate the results of their investment evaluate their success (or failure) and make decisions about future directions. Structured evaluations of a project’s impact are an essential element of the justification for investment in HIT. A systematic approach to evaluation and testing should allow for comparison between different HIT interventions with the goal of identifying and promoting those which improve clinical care or other outcomes of interest. The question of the day is no longer why perform evaluations but how to perform evaluations. This updated book provides an easy-to-read reference outlining the basic concepts theory and methods required to perform a systematic evaluation of HIT. Chapters cover key domains of HIT evaluation: study structure and design measurement fundamentals results analysis communicating results guidelines development and reference standards. Updated case studies and examples are included demonstrating the successes or failures of these investments. The authors also include new initiatives put in place by the government and discuss how they are being adopted and used by health systems. | Health Information Technology Evaluation Handbook From Meaningful Use to Meaningful Outcomes

Process Redesign for Health Care Using Lean Thinking A Guide for Improving Patient Flow and the Quality and Safety of Care

Process Redesign for Health Care Using Lean Thinking A Guide for Improving Patient Flow and the Quality and Safety of Care

Process Redesign for Health Care Using Lean Thinking is a response to a simple but hard to answer question and is the result of the experiences of a working doctor who was also the chief safety and quality officer of an Australian teaching hospital. At this hospital he observed that the Emergency Department was staff by talented well-trained and respected doctors and nurses. The facilities were modern and the work load unexceptional but the department was close to melt down. Bad things were happening to patients everyone was blaming each other lots of things had been tried but nothing was getting better and no one could explain why. The problem was not a lack of technical knowledge or expertise the problem was that no one stood back and said what’s the best way to move 200 or 300 patients a day through the complicated and varying sequence of steps needed to sort out the many different problems that bring patients to our department? These challenges are faced by hospitals and health services all over the world. There are difficulties with patient flow congestion queues inefficient utilization of resources problems engaging clinical staff in improvement programs adverse incidents and budget constraints. Lean thinking and value stream analysis gives hospitals and health services struggling with these issues the insights they need to help themselves. This book provides a method that systematically turns those insights into working programs of service and system redesign. The book is divided into two sections. The first section gives the background to the approach and systematically works through the Process Redesign methodology step-by-step. The second section is a series of case studies that show the methodology in action what worked and what didn’t work. The goal of any process redesign is simple: the right care for the right person at the right time in the right place and right the first time. This book helps the people who work in hospitals and health services realize these goals by working together. | Process Redesign for Health Care Using Lean Thinking A Guide for Improving Patient Flow and the Quality and Safety of Care

American Healthcare Why It Costs So Much Yet Remains a Beacon of Growth and Development

American Healthcare Why It Costs So Much Yet Remains a Beacon of Growth and Development

Selected by the Association of University Programs in Health Administrationfor the Bugbee-Falk Book Award Is the health sector a curse or a blessing? The American health sector now accounts for a fifth of the economy. American healthcare spending per capita far exceeds that of other developed countries. Yet our health as measured by life expectancy and infant mortality is poor by comparison with the developed world. Other measures of quality including hospital-acquired infection are too common. Healthcare costs financially cripple households despite advances associated with the Affordable Care Act. There is widespread dissatisfaction with the American healthcare system and support for more change. It is also the case that the health sector has been a leader in the evolution of the American economy. Economic development is driven by innovative technology. We tend to applaud new technology and the improvement it brings to our lives. Important recent technologies often grow rapidly and faster than the wider economy. This leads to larger shares of the economy. Yet there is considerable apprehension about costs and economic impact of health spending. This book details important health sector institutions and uniquely explores linkages between healthcare and broader economic growth. The book addresses asymmetric information between providers and consumers as well as between insurers and beneficiaries. There is a focus on monopoly power in labor markets which contributes to inefficiencies in the system. The author also discusses cost-effectiveness and allocative efficiency as well as emphasizing productivity and its relationship to the wider economy. | American Healthcare Why It Costs So Much Yet Remains a Beacon of Growth and Development

There is no end in sight to the frequency with which physicians nursing professionals and other healthcare providers will become lawsuit targets in our litigious society. While politicians practitioners insurance companies and trial attorneys debate the nation’s chronic malpractice crisis suits continue to be filed. In addition once COVID-19 is behind us and the unprecedented public support for health care providers wanes as it will it is anticipated that physicians and nurses will become malpractice defendants to a remarkable degree. National legislative fact-finding committees and investigative bodies which may be charged with the responsibility of pursuing a solution likely will never achieve a global remedy. Although curtailed by some states national legislation has not addressed baseless malpractice suites or grossly excessive monetary verdicts. Another approach exists however. Health care providers can impact the existing system and influence the malpractice environments in a tangible positive and powerful fashion. Although there will be debate over tort reform in order to bring some degree of protection to the malpractice defendant individual case success defined from the defendant’s perspective as a no-cause trial verdict can be realized if well-credentialed and experienced health care professionals are willing to assist the malpractice defense bar as expert witnesses. The benefits to the health care community and the individuals who are willing to participate are innumerable and worth considering. | Power from Within A Guide to Success as a Medical Malpractice Defense Expert
